Employee Assistance

Assistance is available to Indiana Department of Correction employees, contract staff members, or their immediate family members to provide short-term immediate monetary assistance should they suffer a serious loss as a result of a natural disaster or catastrophic accident, or an employee dies at the workplace or otherwise while on duty.

The intended purpose of any monetary assistance given from the fund under these circumstances is to ensure employees and their immediate families are able to continue to receive the benefit of the necessities of life immediately following an unexpected catastrophic event.

Note to Applicants: CPAFI funds are not intended to be a replacement for insurance. CPAFI reserves the discretion to offset disbursements or otherwise deny applications for events or distributions which would ordinarily be covered by employment insurance.

CPAFI Assistance Criteria

In Case of Displacement

Temporary

When an IDOC employee's displacement from home is temporary due to fire, flood or other natural disaster. $1000 distributed immediately, with an addition $1000/week displaced up to a four additional weeks. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $5,000

Permanent

When an IDOC employee's displacement from home is (completely) destroyed by fire or natural disaster - $1000 distributed immediately, and another $4,000 provided upon request and submission of W9. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $5,000

In Case of Illness

Dependent or Household Member

Must include hospitalization of the dependent/household member, and will be reviewed and considered by the Board on a case-by-case basis. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $2,000)

Catastropic Illness

To qualify, staff person's illness muct be such that they are hospitalized for a minimum of two days, and that they are off work for a minimum of 10 days. Initially, all applicants would recive $1000. However, if there is a protracted illness and with agreement to submit a W9, the staff member could request up to an additional $3,000 and the approval of any additional amount will be subject to the review and approval on a case-by-case basis by a majority of the Board Members. (cap $4,000

In Case of Injury

IDOC Employee serious Injury during line of duty

Includes any injury that takes place at the workplace or in the persons official duties for the IDOC. Injury must be to the extent it necessitates some hospitalization or absence from work. Employee will receive $1000 immediately, and up to $4000 with a follow up request. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $5,000)

In Case of Death

Death of an Dependent

Flat rate in all cases when Superintendent or Supervisor or facility representative validates. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $2,000

Death of an Financially Contributing Household Member

Could include death of employee's parent or fiance living with employee and providing revenue to the household.

These requests would be reviewed by the Board on a case-by-case basis. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $4,000)

Death of an IDOC Employee (Line of Duty)

$1000 would be immediately distributed to the person overseeing the details of funeral preparation. For tax reasons, the closest next of kin, or legal guardian would be permitted to ask for up to an additional $4,500 to pay for unanticipated expenses. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $5,500)

Death of an IDOC Employee

$1000 would be immediately distributed to the person overseeing the details of funeral preparation. For tax reasons, the closest next of kin, or legal guardian would be permitted to ask for up to an additional $4,000 to pay for unanticipated expenses. Applicant must submit a W-9 prior to disbursement for tax reporting requirements. (cap $5,000)

Discretionary

CPAFI recognizes that on occasion employees may suffer a financial hardship that does not fall into one of the well defined categories of assistance. CPAFI reserves the right to consider all applications for assistance if they fall within the spirit and purpose of the CPAFI mission which is to provide financial assistance to IDOC employees in times of need.

A one time benefit of up to $1000.00 to an IDOC employee on a case by case basis for applications that do not meet the regular criteria for CPAFI assistance but nevertheless do merit some assistance due to specific and well articulated need. Approvals over $500 will only be disbursed upon receipt of a W9 from the applicant.

CPAFI is a 501 (c) 3 charitable fund, who provides short-term immediate monetary assistance to Indiana Department of Correction employees or their immediate family members.

 

Proceeds may also be used for tuition assistance scholarships for employees of the Department or their immediate family members or dependents.

 

All of the funds are administered by the Board of Directors of the Correctional Professionals Assistance Fund of Indiana.
